Because keeping your teeth clean and healthy is such an important part of Orthodontic Care we advise you to visit your dentist every six months for a check up and cleaning. Please print this "DENTAL REWARD VOUCHER" and take it to your dentist appointment, Have them complete the information and we will reward you:
Dental Exam and Cleaning=5 tokens
Dentist-No cavities=5 bonus tokens
Bring can good in at Christmas time= 1 token (limit 10 cans)
Good brusher=1 token
No loose brackets=1 token
Wear your t-shirt to school and then to your appointment=1 token
Celebrate your birthday=2 tokens
Refer a friend=10 tokens
